this form. You will be billed for these services. Your payment must be
received prior to your ad being published.
One modification to your ad after initial
acceptance is at no charge, additional changes will be billed at the rate of
$25 per hour for setup time. You are responsible for obtaining written
permission to use any logos, trade names, trade marks on the website. You are
responsible for submitting all photos/graphics in acceptable digital formats.
